Advocate for Services

Case management supports in assisting the person in managing their life in the community by linking them with supportive services to avoid gaps in care. The case manager advocates and works with the person while providing education and monitoring of progress as recommended in the plan of care by helping them to develop a greater understanding of topics that promote and sustain a life in recovery.
